To fill out this form, you will need to know the following:

• Social Security Number
• Home address history for the past 3 years
• Current driver license number and driver license history for the past 3 years
• Employment history for 10 years
• History of traffic accidents, violations and/or convictions from the last 3 years (including DUI or reckless driving conviction and license suspension)
• Criminal history
• Military history (if applicable)

• Must be between 23 and 67 years old with a minimum of 2 years of verifiable commercial driving experience under the US DOT FMCSRs. Please note – We must be able to contact previous employers to verify this experience, personal experience of owning/hauling a camper does not qualify.
• Class A CDL is preferred but not required – If you do not have a CDL, your license must state that you are licensed to transport a commercial load with a GVWR of up to 26,000 lbs.
• Must be able to read and speak English.
• Full 3 years of work history for Non-CDL, 10 years for CDL, must be documented on the application with contact names, phone numbers and employment dates.
• Must meet the physical requirements of 49 CFR 391.41 and have a current DOT Medical Examiner’s Certificate.
• Pass a Pre-Employment Drug Screening prior to being contracted.
• Get pre-approval through INTransport’s internal Insurance Provider and meet the requirements for Commercial Unladen Coverage through an INTransport approved provider (see below for further detail).
• An MVR Report will be run for the previous 3-7 years of driving records. Drivers with more than 3 moving violations and/or a total of 4 or more points will not be approved.
• A PSP Report will be run and approved or denied (PSP Reports include all Commercial Motor Vehicle Inspections and Accidents).
• Must have a working smart phone (Android or iOS) with an active data plan, working camera and voicemail.
• All cellphones must meet the Handsfree/Bluetooth requirements as set by the US FMCSA/DOT.
• Must be proficient in email and industry specific apps, I.E. Drive Axle, EFS, and EL Logging.
  • If using a tablet for EL Logging, tablets must have an active data plan, with data on at all times to keep logs current.
• Have a valid passport for any desired Canadian trips.
• All Non-CDL Drivers must complete and pass an INTransport Road Test at Orientation.

• Drivers must be pre-approved through INTransport’s internal Insurance Provider.
  • No history of drug, DUI, OWI, careless, reckless driving, or habitual offender convictions.
• Drivers must be eligible for Commercial Unladen Coverage through an INTransport approved provider.
  • Drivers must be between the ages of 23 and 67 years old (unless agreed upon in advance by the Insurance Company’s Underwriters).
  • Drivers with any major violations or convictions at any time in the past will not be eligible. Such violations/convictions include but are not limited to:
    • DWI, DUI and/or any drug related conviction
    • Any felony convictions
    • Manslaughter/negligent homicide or hit and run
    • Racing or reckless driving violations/convictions
    • Driving on a suspended license
    • Fleeing/eluding arrest

In connection with your application for employment (including contract for services), you understand that the consumer reports or investigative consumer reports which may contain public record information may be requested or made on termination of employment, work experiences, accidents, safety performance, etc. Further understand that such reports may contain public record information concerning your driving record, workers' compensation history, credit, bankruptcy proceedings, criminal records, etc. From federal, state, and other agencies which maintain such records.

By signing below, you hereby authorize without reservation, any party or agency contacted by this employer to furnish the above mentioned information. You further authorize ongoing procurement of the above mentioned reports at any time during your employment (or contract). You also agree that a fax or photocopy of this authorization with your signature may be accepted with the same authority as the original.

You have the right to request to Intransport, LLC upon proper identification and the payment of any legally permissible fees, or the information in its files on you at the time of your request. You hereby authorize and request, without any reservation, any present or former employer, school, police department, financial institution, division of motor vehicles, consumer reporting agencies, or other persons or agencies having knowledge about you to furnish Intransport, LLC with any and all background information in their possession regarding you, in order that your employment qualifications may be evaluated.

In conformity with 49 C.F.R. Part 40, I hereby authorize the carriers (company/school) listed on my applications to furnish to Intransport, LLC on behalf of the Company listed below the following information concerning drug and alcohol tests, DOT drug and alcohol testing violations including pre-employment tests during which the past three years (I) the dates on which I tested positive for drugs and the drugs involved; (II) the dates on which I tested .04 or greater for alcohol and the test result levels; (III) the dates on which I refused to be tested for drugs and/or alcohol; (IV) any failure to undertake or complete a rehabilitation program prescribed by a Substance Abuse Professional; (V) other violations of DOT drug and alcohol testing regulations; and (VI) any information that carriers have received regarding violations of drug/alcohol testing regulations from my previous employers observed by DOT.

A summary of your rights under the Fair Credit Reporting Act is being provided to you separately.

I fully understand that the information I authorize Intransport, LLC to receive involves tests which were required by the Department of Transportation (DOT). If any carrier (company/school) listed on my application furnishes Intransport, LLC with information concerning items (I) through (VI) above, I also authorize that carrier (company/school) to release and furnish the dates of my negative drug and/or alcohol tests and/or tests with results below 0.04 during the three-year period and the names and phone numbers of any substance abuse professional who evaluated me during the past three years.

You are entitled to a free disclosure if: a person has taken adverse action against you because of information and your credit report; you are the victim of identity theft and placed a fraud alert in your file; your file contains an accurate information as a result of fraud; you are on public assistance; you are unemployed but expect to apply for employment within 60 days. In addition, all consumers are entitled to one free disclosure every 12 months upon request from each nationwide credit bureau and from nationwide specialty consumer reporting agencies. You have the right to ask for a credit score.Credit scores are numerical summaries of your credit-worthiness based on the information from credit bureaus. You may request a credit score from consumer reporting agency's that create scores or distribute scores used in residential real property loans, but you will have to pay for it. In some mortgage transactions, you will receive credit score information for free from the mortgage lender.

You have the right to dispute incomplete or inaccurate information.If you identify information in your file that is incomplete or inaccurate, and report it to the consumer reporting agency, the agency must investigate unless your dispute is frivolous. Consumer reporting agencies must correct or delete inaccurate, incomplete, or unverifiable information.Inaccurate, incomplete or unverifiable information must be removed or corrected, usually within 30 days. However, a consumer reporting agency may continue to report information it has verified as accurate.

Access to your file is limited.A consumer reporting agency may provide information about you only to people with a valid need — usually to consider an application with a creditor, and sure, employer, landlord, or other business. The FCRA specifies those with a valid need for access.

You must give you consent for reports to be provided to employers.A consumer reporting agency May 9 give out information about you to your employer, or a potential employer, without your written consent given to the employer. You may limit "prescreened" offers of credit and insurance you get based on information in your credit report.Unsolicited "prescreened" offers for credit and insurance must include a toll-free phone number you can call if you choose to remove your name and address from the lists these offers are based on. You may opt-out with the nationwide credit bureaus at 1-888-567-8688.

You may seek damages from violators. If a consumer reporting agency, or, in some cases, a user of consumer reports or eight furnisher of information to a consumer reporting agency violates the FCRA, you may be able to sue in state or federal court.

States may enforce the FCRA, and many states have their own consumer reporting laws. In some cases, you may have more rights under state law. For more information, contact your state or local consumer protection agency or your state Attorney General.

I, the proposed applicant listed below, hereby acknowledge that, in connection with my application and/or appointment to driving responsibilities, INTransport, LLC will procure a Motor Vehicle Report (MVR) on me as part of their consideration of my application and to determine whether I meet its risk management standards for insurability on their commercial auto insurance policy. In the event that information from the report is utilized in whole or in part in making an adverse decision, INTransport, LLC well upon my request cause to have mailed to me, by U.S. first class mail to the address on my driving record, a copy of the MVR which has been utilized. I have been provided a summary of my rights under the FCRA.

I understand that I have the right to request in writing within a reasonable time that INTransport, LLC make a complete and accurate disclosure of the nature and scope of the information requested.

This release and authorization shall remain valid and in effect for the duration of my contract, initial employment or subsequent employment with INTransport, LLC, it's affiliates and/or its subsidiaries. INTransport LLC reserves the right to run subsequent MVR’s on me as INTransport, LLC in its sole discretion determines is necessary.

I may revoke this Authorization at any time, provided that the revocation is in writing, except to the extent that INTransport, LLC has taken actions relying on this Authorization. If I proceed with such revocation of this Authorization, I will send a written revocation to INTransport, LLC at their address at which this application has been made.

By signing below, I hereby authorize INTransport, LLC and its affiliates or subsidiaries to obtain a Motor Vehicle Report about me in order to process my application or driving appointment by evaluating my driving insurability on their commercial and auto insurance policy. In addition, I also authorized all entities having information about me, including departments of motor vehicles, to release such information to INTransport, LLC and its affiliates or subsidiaries.

AUTHORIZATION

If you agree that the Prospective Employer may obtain such background reports, please read the following and sign below:

I authorize INTransport, LLC ("Prospective Employer") to access the FMCSA Pre-Employment Screening Program (PSP) system to o seek information regarding my commercial driving safety records and information regarding my safety inspection history. I understand that I am authorizing the release of safety performance information including crash data from the previous five (5) years in inspection history from the previous three (3) years. I understand and acknowledge that this release of information may assist the Prospective Employer to make a determination regarding my suitability as an employee.

I further understand that neither the Prospective Employer nor the FMCSA contractor supplying the crash and safety information as the capability to correct any safety data that appears to be incorrect. I understand I may challenge the accuracy of the data by submitting a request to https://dataqs.fmcsa.dot.gov. If I challenge crash or inspection information reported by a State, FMCSA cannot change or correct this data. I understand my request will be forwarded by the DataQs system to the appropriate State for adjudication.

I understand that any crash or inspection in which I was involved will display on my PSP report. Since the PSP report does not report, or assign, or imply fault, I acknowledge it will include all CMV crashes where I was a driver or co-driver and where those crashes were reported to FMCSAm regardless of fault. Similarly, I understand all inspections, with or without violations, will appear on my PSP report, and State citations associated with FMCSR violations that have been adjudicated by a court of law will also appear, and remain, on my PSP report.

This certifies that this application was completed by me, and that all entries on it and information in it are true and complete to the best of my knowledge.
